Payment is due before the training.

Training sessions are for limited attendance. Please make reservations early.
Registration for training closes
one week prior
to the scheduled date.
Confirmation of registration will be e-mailed upon receipt of this form, along with travel and accommodation information.
There is a $200 cancellation fee if cancellation is within one week of training.
